New Patients

Please arrive 15 minutes before your appointment to complete the new patient paperwork.  You will sign a consent for services, acknowledgment forms, and authorization to release your health information. You will also review our Notice of Privacy Practices and will be provided with a copy if requested.

When attending your first visit, please be sure to bring:

• Insurance card

• Driver’s License or photo identification

• All current medications you are taking or completed medication list form.

• Completed health history form (if requested)
